How do I order a new Welfare ID Card?

Contact the Fund Office and ask to speak with an Accounts Receivable Specialist. They will verify eligibility and request a new Welfare ID card. Your new ID card will arrive within 7-10 business days from the date you notify the Fund Office that you need a new card.

How do I locate a Welfare network provider?

The Service Providers page provides links to the Welfare service providers BlueCross BlueShield of Kansas City, Connection Dental, Sav-Rx, and New Directions. Once you have selected the desired provider site, you may login to your account, if applicable, and search for in-network providers.

How do I view my Explanation of Benefits (EOB)?

If you would like to view your Medical EOBs, please visit the BlueCross BlueShield of Kansas City website. Once you are on the BCBS website, you will need to login to your account to access your EOBs.

How do I file an out-of-network health claim?

If you receive services from a non-participating provider and they are unable to submit the claim on your behalf, then you may have to submit the claim to the Plan. If you need to submit a claim, you will need to request an itemized bill from the provider then complete a Member Reimbursement Claim Form. Forward the itemized bill and completed form to the address as it appears on the claim form.

How do I designate a beneficiary?

Complete a Beneficiary Designation Form and mail to the Fund Office.

How do I add a dependent to my health insurance?

To enroll your spouse for coverage under the Plan, submit a completed Information Verification Form along with a copy of the marriage certificate to the Fund Office. Make sure to include all dependents on the form or it will not be accepted.

To enroll your dependent child for coverage under the Plan, submit a completed Dependent Child Enrollment Form or Information Verification Form along with a copy of the birth certificate or adoption papers to the Fund Office. If you are not married to your child’s birth mother/father, then you would also need to complete a Dependent Affidavit Form, have it notarized, and submit it to the Fund Office with any supporting documents.

How do I file for an extension of my health coverage if I am unable to work due to a disability?

Submit an Initial Disability Claim Form to the Fund Office. For additional update requests, you may complete the Supplemental Disability Claim Form. If you are disabled on Workers Compensation, you must submit Proof of Compensation and be unable to engage in any occupation or employment for wage or profit.

How can I find more information about the health benefits offered through my health plan?

The Summary Plan Description is the document that details the benefits of the health plan. The SPD will provide information about the health plan such as the applicable co-insurance amounts, deductible amounts, and out of pocket maximums.

When do I receive my Welfare ID card?

Once you become eligible for benefits, you will receive your Welfare ID card within 7-10 business days.

How do I notify you if I become eligible for other insurance?

To update your other insurance information, submit a completed Information Verification Form. You may complete the form, by clicking the link. Make sure to list all eligible dependents, whether they are covered under the other insurance or not, or it will not be accepted.

How do I let you know if my other coverage has terminated?

If your other coverage has terminated, we will need a copy of the Certificate of Creditable Coverage (COCC) sent to you by the other insurance company. You can mail the Certificate to the Fund Office.

How do I get the Welfare Summary Plan Description Booklet (SPD)?

You can review the Summary Plan Description electronically or you can call the Fund Office to request a hardcopy of the SPD Booklet.

If I get divorced, what happens with my former spouse’s coverage?

You or your spouse must notify the Plan and mail a fully executed copy of your divorce decree to the Fund Office. Once the Plan receives the divorce decree, your former spouse’s coverage will be terminated as of the date of your divorce. You may also want to submit an updated Beneficiary Designation Form.

When I retire, what are my options for continuing health coverage?

If you meet certain criteria defined in the Welfare Summary Plan Description you and your family may be eligible to participate for regular retiree plan or retiree Medicare supplement plan. Please contact the Fund Office for your retiree options for continuing health coverage.

How do I apply for my pension?

Complete an Application for Retirement Benefits. You should apply for your pension at least 90 days prior to your retirement date. Please complete the forms and mail along with any noted documentation to the Fund Office.

How do I sign up for Direct Deposit for my pension payments?

Complete a Direct Deposit Form and return it to the Fund Office.

How do I change my pension payment tax withholdings?

To change the amount of the taxes withheld from your pension payment, complete a Tax Withholding Form and return it to the Fund Office.

What happens to my pension if I pass away prior to receiving benefits from the Plan?

Your surviving spouse or beneficiary may be entitled to a Death Benefit if you pass away prior to receiving benefits from the Plan. The form, amount and commencement date of the benefit payments depend upon various factors, including the date of your death.

What happens to my pension if I get divorced?

If you divorce (whether before or after retirement), your spouse may be entitled to receive a portion or all of your pension benefit in accordance with the terms of a Qualified Domestic Relations Order (QDRO). Under the terms of a QDRO, certain payments could be made from your benefits to pay alimony, child support, or marital property rights to your former spouse, child, or other dependent. If you divorce, you must contact the Fund Office to ensure your benefits are paid properly. You may also want to submit an updated Beneficiary Designation Form.

A QDRO may affect the amount of pension benefit you will receive or are receiving. A copy of the Fund’s procedures for handling QDROs will be provided to you, free of charge, upon request. If you have questions about QDROs, please contact the Fund Office.

What contributions fund my account?

Participating employers contribute to the Plan on your behalf pursuant to the terms of a collective bargaining agreement (CBA) with the Union. The CBA tells the employers how much to contribute on your behalf.

The employer contributions made for you go into an account, referred to as a “Participant Account”, established and maintained by the Trustees.

You will automatically become a Participant on the first day on which an Employer is required to contribute to the Plan on your behalf. Once you become a Participant, you will remain a Participant until your Participant Account is distributed either upon retirement, disability, terminated work for all contributing Employers or death. You are fully vested in your Participant Account at all times.

Can I contribute to Plan?

You are not permitted to make voluntary contributions on your own behalf.

When can I withdraw funds from my account?

You are eligible for payment of your Accounts upon:

Attaining Normal Retirement Age (age 62).
Attaining Early Retirement Age (age 55-61) and withdraw from all employment with a covered employer or Disqualifying Employment.
Disability. Receipt of a Social Security disability award or with 2 independent doctors reports stating that you are totally and permanently disabled.
Small Account Balance – if your Participant Account Balance is less than $5,000.00 and you have not had contributions to the Plan for 2 Plan Years
Special Rollover Distribution – if your Participant Account Balance is greater than $5,000.00 and you have not worked in Disqualifying Employment for 5 consecutive Plan Years and are withdrawn from all employment with a covered Employer.

How do I apply for a distribution from my account?

To apply for a withdrawal, you will need to complete an Application for Benefits. Please complete the form and mail along with any noted documentation to the Fund Office.

Can I take a loan from my account or are there hardship distribution options?

The plan does not provide financial hardship or other payments before the eligibility dates. The plan does not provide loans from your Accounts.

Is the Supplemental Plan benefit taxable?

The payment you receive from the plan will be subject to mandatory withholding of 20% for federal income taxes. You can avoid mandatory withholding only if you arrange to have your benefit transferred directly to an IRA or an eligible retirement plan.
